Adoption Medical History Registry

 

What is the Registry?

The Registry is a means for birth parents to share family medical history with the child they have placed for adoption.

Who does the Registry server?

The Registry serves the birth parents who gave birth in Pennsylvania and who released their child for adoption and the Pennsylvania born adoptees.

Who may file medical history information?

A birth mother or birth father may file medical history information.

Who may request the medical history information?

Pennsylvania born adult adoptees, or adoptive parents or legal guardians of Pennsylvania born minor adoptees.

Will the privacy of individuals participating in the Registry be protected?

Yes. Medical history information released to the adoptee will be limited to non-identifying information only. Neither a birth parent’s name nor any other identifying information will be provided.

Is there a fee?

No.
